Needs Assessment Survey
As the new Agriculture Agent for Polk County, I’m interested in learning about what topics and areas you want more information about. By letting me know what topics are interesting to you, I will be better able to develop and organize educational programs and activities which directly benefit you and your operation.
Please download and fill out the 2011 Needs Assessment Survey (MS Word Doc, 2 pages, 47 KB) and then either save a copy to your hardrive and email it to me (jennifer.blazek@ces.uwex.edu) or print it out and mail it to the Polk County Extension Office. Return surveys before August 30th!
**Everyone who returns a survey by that date will be entered in a drawing to win a new 2011 County Plat Book or $25.00 towards Extension programming or publications, whichever you prefer.** ________________________________________________________________________STAFF
